Forms

The following documents were created or obtained from various sources concerning inspections of construction sites, dry weather field screening (DWFS), illicit discharge detection and elimination (IDDE) inspections, and municipal facility inspections.  There are no regulatory requirement to use a specific type of form.  Each permittee can select portions of many forms to create a customized form for their city, or print one of the forms as is for local use. 

Those forms and guidance documents that were received or created by INCOG in Word document format are left in Word to help with editing and customization.  However, most forms obtained by INCOG were in pdf format, and these will have to be redone in an editing software such as Word or Excel for local customization.
